OSP Mixer ATA Road case. I believe this is a MC12U-4
MC12U-4 4 Space Amp Rack with a 12 Space Slanted Mixer Top Rack. 12 Spaces of rack space on top for mounting your rack mountable mixer. This unit has rubber feet but no casters. Dimensions pictured below.
ATA cases for standard 19" rackmountable gear with a 12 space slanted top-mounted rack for 19" rackmountable mixers. Our MC series cases feature removable front, top and back covers. Ball corners, recessed spring-loaded handles, and aluminum edges on all models.

We bought this a few years back locally from Stage Sound in Roanoke VA, and we never used it. It's been sitting in the warehouse safe and protected, just some dust and maybe a touch of tarnish. It's is an extremely well built road worthy case with heavy duty spring-loaded and tight fit corners. I'm not sure it's ever been opened until I took these photos.
