Dual Tremolo - handpainted from JHS. When the green LED is on, the Green knob controls the speed of the trem, when the red LED is on the red knob controls the speed of the trem. Works perfectly. The paint on the bottom came off on some previous velcro, but it's not visible when it's on the board. Love this pedal, but I just don't need it anymore. No box or power supply.
