This was my first midi interface. I bought it and a Mac Quadra 650 with Studio Vision in 1995. I used it for many years and only retired it when I no longer had a machine with a serial port or the ability to add a serial port. I don't have any way to really test it now, but it powers up and it was working perfectly when I retired it.
The original power supply and manual are included.
Edit: I also just found a pair of serial cables and will include them as well.
