In great shape and sounds wonderful. This is a very late V3, though the PCB is labelled EH 5150 Rev D, which in most cases was the green version and sometimes was a V4 (which used a 5106 Reticon chip for the effect instead of the SAD1024). Has the post-1980 "hockey puck" style control knobs, but the layout of the board and chips is definitely V3, as is the enclosure. Did not work when I got it. The caps were just getting out of spec, but none were dead. Did a recap. In the end this needed a recalibration as someone previously had done some serious meddling with the 4 internal trimmers. Fortunately these are fairly easy to calibrate by ear. I marked the trimmers in there, but you shouldn't have to touch them now. The bottom plate screws and one of the vinyl feet are replacements.

For those not familiar with these, the slider switch for "Filter Matrix" essentially turns off the LFO and lets you set the envelope by hand, much like a cocked wah but with all the added richness of the Electric Mistress circuit added in. In Matrix mode just turn the Color knob all the way up and use the Range control knob to find the sound you want and leave it there. A nice feature. Works perfectly and sounds amazing.
