This is the 4000XL Buffer/Preamp for use with the Barcus Berry contact piezo pickups (pickup not included). 1/4" to 1/8" cable is included.
It has some surface wear as shown, but works just fine, and it's priced accordingly (these can typically sell for quite a bit more).
