Its a 1979 Gibson ES Artist Fact Sheet. Its 8 1/2 inches by 6 inches. It has some cardboard backing and encased in a plastic protection sheet.
We got this from a since closed guitar shop.
Very cool Gibson item that you just don't see anymore...
