Used DLS Chorus-Waves pedal, no box. Works perfectly.
This pedal does a subtle chorus very well when using the blend knob to bring your clean signal back in parallel with the effected signal. It has a variable width knob to select different types of chorusing.
The finish has a couple small spots where the coating has chipped to show the raw enclosure beneath (represented in the photos).
