1990s Electro-Harmonix Micro Synthesizer Pedal.  Includes Power Supply and original wood box.  The paperwork includes user manual and also suggested presets.  Dial up some great Smashing Pumpkins and Muse tones.  Has a beefier sub octave and tracks a bit better than the attached cable version.  

The Electro-Harmonix Micro-Synthesizer is a powerful guitar processor which creates lead synthesizer voicings, as well as exciting simulations such as steel guitar, organ, and violin. Four completely independent and continuously mixable voices-Guitar, Octave above, Sub-octave, and Square Wave-are modified with envelope and multiple filter controls to achieve a flexibility and variety of sound. Rear Trim pot adjusts the preamp gain.
