Made by Warmoth. I was going to use this crazy looking thing for a partscaster I was trying to make, but I never committed to this Frankenstein style pickguard. Still has the protective warranty plastic on it (warranty is expired).
The configuration is an angled Humbucker in the bridge position, a Strat style single coil in the middle, and a P-90 in the neck position.
Crazy but pretty cool looking.
