This a used but in good shape SLO style headshell made by Weber. At some point the grill supports were trimmed up to allow a taller chassis so the bottom two screws holes are gone. I just have the three top ones in and never it's bothered me. Fits a 22 x 8 x 2-1/2" Chassis. Inside vertical clearance is 7-1/4".
