This is a Dingotone CSV Classic Seventies Fuzz. It is in good condition, but is a little beat up and has a couple strips of Velcro on the bottom, plus residue from a larger piece of Velcro that was attached some time in the past.
I went through a fuzz collecting period about 10 years ago, and this is one of the ones that doesn't get much use, so my loss is your gain.
