This is a very rare acoustic Melobar. A Melobar is a slide guitar with the neck set at an angle so it can be played standing up. 

While he electric Melobars are relatively common, the acoustics are quite rare. While it is still perfectly playable it's got more then a few dings. Due to the stress of ten strings on the neck the neck has needed to be repaired. It has also been reinforced on the inside to help cope with the stress. I would recommend detuning when not playing and keeping it well humidified. Also a previous owner upgrade the tuners at some point. 

Ted Smith (the melobar inventor's son) messaged me this info on the guitar. I apparently had the wrong model info up originally. 

Built in the mid-seventies these were a modified 12-string acoustic with an aluminum angled neck bolted to an existing neck. This is an excellent example of on in good condition. Most self destructed over the years from the 10 string pressure put at an angle and if you own one I'd string it six string to keep it from imploding over time. Steven Sherman Smith built most of these with Doug Smith for a Melobar teaching studio in Meridian Idaho that was attempted for a couple of years.
