This is a rare piece. Rare enough that I can’t find any information about it at all. It works and sounds fantastic though. Looks to be custom made. 3 bands of sweepable eq with wide and narrow Q. Sounds as good as any vintage console EQ I’ve ever used. Just cleaned and serviced, ready for installation.
Low band - 30Hz to 320 Hz +/- 15db
Mid band - 260Hz to 5KHz +/- 15db
High Band - 2KHz to 15KHz +/- 15db
Top knob is overall gain
