This is an Echo AudioFire 12 Firewire audio interface. It's a really nice unit if you want to add lots of I/O to your setup, whilst being compact enough to occupy a single 1U Rack space. You could use it to capture audio from several sources for multitrack recording, or run out the stems from your DAW to mix down through an analog desk, or bounce down to tape.
I've used this on Mac OS Sierra in both Ableton & Logic, where it has performed admirably, including creating an aggregate device with a separate USB interface. You can also use it as the Master Midi clock for your setup. Additional units can be daisy-chained using the second Firewire port.
You have full control over routing and levels using the Echo Firewire Console (pictured), these settings can be flashed onto the interface if you are using it standalone (e.g. jamming outside of a DAW).
The inputs all support balanced input via TRS cable, whilst power is via a standard kettle lead. I will include a power lead and a Firewire 6-pin to 9-pin cable to connect the interface to a computer.
This unit functions perfectly, it has a bit of rack rash on top as pictured but nothing you would see in a rack anyway. It's really solidly constructed and also includes the original box pictured.
