Simple and effective mono preamp unit
From my personal studio, used exclusively on a 57 for the duration of its life. Very clean and well-taken care of, only a couple of scratches that are unnoticeable when racked (one scratch next to the logo on the face). Free of rack-rash, as far as I can tell. Looks and sounds great.
Recently serviced, cleaned circuit contacts (no scratchy pots/jacks) and tested for full functionality.
Uses a single 12AX7 tube. It's just the unbranded one that came stock, but you could always swap it with a name-brand variation if you think it'll give a different color.
I believe that the additional screw-holes on the underside are for grounding other gear
Not much else to add. It sounds great and adds some subtle character to your tracks. The limiter and VU meter are nice additions. Otherwise, it has everything you'd expect from a preamp. Gives you way more gain than your average half-rack-unit or cloudlifter.

Here's some details that I pulled from it:
• Hand selected 12AX7A vacuum tube circuitry provides a big, warm sound for microphones and instruments that will perfectly fill out your sound for recording
• Selectable output limiter circuitry to avoid clipping and protect connected equipment such as processors
• Convenient analog VU meter allows excellent control of desired signal levels and also monitors the limiter function when the limiter is activated
• Variable Input and Output Gain Controls with over 94 dB of available gain
• +48VDC Phantom Power Switch
• Phase Reversal Switch
• Servo-balanced XLR and unbalanced 1/4" TS Inputs and Outputs
• Rugged construction for maximum long term reliability
• Shielded internal dual regulated power supply with ~115V(60Hz)/~230V(50Hz) select switch and fused IEC power cord connector
